Working with Barbara over email, we discovered that she'd created a 2nd (empty) dataset with a similar name over a year ago, and accidentally deleted the dataset with data in it.

We were able to restore the majority of the data from a backup.

Sorry it took us so long to get back to you. I fixed your problem so that you can add columns again. In the process I added a column "Notes" to your dataset; feel free to use it, rename it, or delete it as necessary.

Assuming you're the owner of a dataset, you can change the title by going to the "Summary" tab below the the dataset, and clicking on the title to edit it:
